The reason is that you need the code ready before you attempt to login. It cannot be one that reacts to a prompt and you cannot use SMS. LastPass provides a user-friendly web vault for Windows and Mac, a desktop application for Windows only, and a Safari-based app for Mac. The LastPass Duo multifactor window is displayed after username and password. The standard version of LastPass comes with a web interface, but also includes plugins for various web browsers and apps for many smartphones. The workaround requires that you use a basic MFA code generator app (LastPass Authenticator, Microsoft Authenticator, Google Authenticator, etc.) that generates new codes every 30 seconds. The LastPass for Applications program is available for Microsoft Windows only. User reports indicate no current problems at LastPass LastPass is a freemium password manager that stores encrypted passwords online. This is a code bug even if LastPass won't admit it. Uninstalling and reinstalling or resetting the app will not help. It's another couple of years later and the issue is still not resolved, but I have a (very annoying/challenging) workaround - but it's not a fix.
